Cost & Structure
How much does physical therapy marketing cost?
Launching a new clinic? A website plus SEO to get found on Google and AI search typically runs $1,000–$2,000/month. It only takes 1–2 new patients a month (at a $1,000 lifetime value each) to cover that cost.
Growing past 5 providers or adding a second location? Plan for $2,000–$3,000/month. That covers your website, SEO, and Google Ads. About 3 new patients a month pays it off.
Scaling to multiple locations? Budget $3,000–$4,000/month. That covers website, SEO, Google Ads, per-location marketing, plus social and email. Just 4–5 new patients covers the cost.
Already an enterprise brand with 5+, 20+, or even 250+ locations? Expect to invest more overall, so every location performs. But you’ll actually pay less per location than a smaller clinic does.
Is it cheaper to hire a Marketing Coordinator or work with a physical therapy marketing agency?
A full-time Marketing Coordinator runs about $60,000/year plus benefits — roughly $5,000+/month. Compare that to $2,000–$3,000/month with a physical therapy marketing agency, and you’re getting a full team (Customer Success Manager, Google Ads Specialist, SEO Specialist, social media specialist, and graphic designer) for less than the cost of one employee.
We love partnering with clinics that have an in-house Marketing Coordinator to own the strategy – but if you’re weighing an agency against a new hire, an agency delivers more expert support at a much lower cost.
Is this a monthly retainer, or one-time project pricing?
We run monthly marketing subscriptions rather than one-time projects, because physical therapy marketing isn’t a “set it and forget it” job. Google’s algorithm and AI search are constantly evolving, so your SEO needs ongoing attention, your website needs regular updates (new locations, providers, etc.), and social media, Google Ads, and email marketing all require monthly effort to work.
Your monthly fee also offsets the big upfront cost of website development.
We offer 12-month and 24-month contract options — sign a 24-month term and we’ll build your website free, saving you $200–$300/month.
Are there setup fees in addition to the monthly cost?
No setup fees, ever. Website setup is fully included in your monthly cost.
Do you require a long-term contract, or can I cancel anytime?
We offer 12-month and 24-month contract options. After your initial term, your agreement renews in 3-month increments, and you can cancel anytime with 1 month’s notice before renewal.
What's included at each pricing tier?
Platinum — Website, SEO, AI search optimization, a dedicated Customer Success Manager, and monthly reports.
Pro — Everything in Platinum, plus Google Ads management from a dedicated Specialist experienced in PT and physio campaigns.
Ultimate Patient Engagement System — Everything in Platinum and Pro, plus done-for-you social media, email marketing, digital newsletters, optional print marketing, and a dedicated Patient Engagement Specialist.
Enterprise — Everything in the Ultimate Patient Engagement System, plus a VIP Customer Success Manager and assistant, systems built to scale across 5+ locations, and complex multi-location/multi-brand website design.
Do prices vary based on clinic size or number of locations?
Yes — we charge an extra $175 per location starting with your second location. Each location needs its own Google Business Profile, website page, and Google Ads campaigns, so we market every location like its own business.
Is there a minimum Google Ads spend required, and is that separate from your fee?
Yes, Google Ads spend is paid directly to Google and is separate from our management fee, which is already included in your monthly subscription. We typically recommend $500–$1,000/month per location — though you don’t have to run ads everywhere at once. Many clients rotate ad spend between locations, which is a great way to give a new or struggling location a boost.
Do you offer any new patient guarantees?
Yes! On our Pro and Ultimate plans, we guarantee 20 qualified new patient calls per month. From there, it’s up to your front desk to answer and get them on the schedule. If we don’t hit that number, your next month is free.
AI can build me a website today. Why should I work with a physical therapy marketing agency?
AI can spin up a website that looks decent — but looking good and ranking on Google are two very different things.
Without real SEO work behind it, an AI-built site has no way to actually get found by patients searching for care.
That’s the gap a physical therapy marketing agency fills: Practice Promotions websites book appointments at 4x the industry average, because they’re built and optimized by a team that knows how to turn traffic into booked patients — not just page views.
Value & ROI
How soon can I expect to see new patients or leads?
Your new website and Google Ads campaigns launch within 8 weeks, and many PT owners are surprised by how quickly the phone starts ringing after that. Physical therapy marketing does take a little time to build momentum — but you should expect full results, including a steady stream of new patient calls, within 6 months of launch.
How do you measure ROI on my marketing spend?
We calculate ROI based on the lifetime value of each new patient you bring in. Using your reimbursement rate — say, 10 visits at $100 per visit, or $1,000 per patient — we can show exactly what your marketing spend is generating. For example, if you’re on our Pro plan at $2,095/month plus $1,000 in Google Ad spend, that’s $3,095 in total investment. Attract 15 new patients from that, and you’ve generated $15,000 in revenue against $3,095 in spend — a $12,000 return, or roughly 5x ROI. Your live dashboard tracks this in real time, so you always know exactly how much your physical therapy marketing is earning you.
What results have other PT clinics seen?
Real clinics, real growth. Alpine Physical Therapy had been stuck at the same revenue level for years — after joining our Ultimate plan, they jumped from 6% to 20% annual revenue growth in just 6 months, with 111 extra patient calls from Google. Platinum PT’s owner planned to try us out and cancel — instead, he was floored by a 10x increase in patient calls (30 to 298 per month over two years), grew to 3 additional locations, and hired 17 new staff. And Pacific Pro PT, a 17-location practice that used to run its own SEO and Google Ads, saw a 5x increase in appointment requests after handing it over to us — plus 22 qualified calls a month and the #1 ranking for “physical therapist” across their service areas.
Working With Your Marketing Team
Do I own my website and content if I stop working with you?
Yes, 100%. We hope you’ll stick around, but if you ever decide to part ways, your website and content are yours to keep — no hostage situations, no hoops to jump through. We’ll help make the transition smooth and stress-free, because we’d rather earn your business every month than lock you into it.
Will I get a dedicated account manager?
Every new client gets a website development specialist and onboarding manager from day one, plus ongoing support that scales with your plan:
Platinum clients get a dedicated Customer Success Manager.
Pro clients get a Customer Success Manager and Google Ads Specialist.
Ultimate clients get a full team — Customer Success Manager, Google Ads Specialist, graphic designer, and Patient Engagement Specialist. You’ll always know exactly who to call.
How often will I get reports on performance?
You’ll never be left wondering how your marketing is performing. That includes monthly meetings with your Customer Success Manager to review results and ROI, monthly reports from your Google Ads Specialist, and monthly check-ins with your Patient Engagement Specialist on social, email, and newsletter strategy.
Every quarter, your whole team gets together with you to plan ahead — new locations, new providers or specialties, seasonal campaigns, and community events all get built into the strategy.
How will I communicate with my team?
Everything runs through Basecamp, on desktop or mobile, so all your conversations, files, and updates live in one place instead of getting buried in email. Have a question or need something adjusted? We respond within 1 business day or sooner — most of the time, faster.
Logistics
What happens if I'm not happy with results?
Reach out to your Customer Success Manager on Basecamp — but honestly, we’re probably already on it.
Our team is bonused on your lead performance, so your results are directly tied to ours. We’ll review what’s happening together, whether that’s on Basecamp or during your monthly meeting, and make changes to get your campaigns performing.
Simply put: we get paid when you win, so keeping you happy isn’t just good service, it’s good business for us too.
Is there a guarantee or trial period?
We don’t offer a trial period, but we do offer something better: a real guarantee.
On our Pro and Ultimate plans, we guarantee 20 qualified new patient calls per month. From there, it’s on your front desk to answer and get them booked. If we don’t hit that number, your next month is free.
What if I already have a website – can you work with it, or do I need a new one?
We always build a new website, and there’s a good reason for that. A physical therapy website needs a strong SEO foundation and a design built to convert visitors into booked patients — and most existing sites simply weren’t built with either in mind.
Rather than patching together an older site with unknown SEO history and structural limitations, a fresh build lets us start you off on solid technical footing, optimized from day one to rank on Google and turn traffic into calls. It’s the fastest, cleanest path to real results.
How long does onboarding take before things go live?
Onboarding moves fast. In the first 4 weeks, we’re deep in discovery and strategy — learning your practice, providers, and goals, and building out your new website and campaign foundations. By week 8, your new website is live and your Google Ads campaigns launch. By week 12, we’re reviewing early performance data and fine-tuning your campaigns and content based on real results, so your marketing keeps improving as it matures.
Do I need to sign anything before pricing is finalized?
Nope. After your first call, we’ll send you a full proposal with transparent pricing based on your practice’s needs. You’ll see exactly what you’re paying before you ever sign anything.
How is pricing different for multi-location practices?
We charge an extra $175 per location starting with your second location, since each one needs its own Google Business Profile, website page, and Google Ads campaigns — we treat every location like its own business. For practices with 5+ locations, our sales team will put together a custom price that reflects your scale. We work with multi-location practices of all sizes, including clients running 20, 100, and even 300+ clinic locations on our Enterprise plans.
Can I upgrade or downgrade plans later?
Yes to both. We hope you never want to downgrade, but if you do, you’re free to move to a lower plan after your current contract term with 1 month’s notice.
Upgrades are even easier — plenty of our clients start on one plan and grow into another. Want to add Google Ads with Pro, or social, newsletters, and email with Ultimate? We’ll kick off your upgrade right away, typically within a month.
How do I get started?
Getting started is simple.
First, schedule a PT Marketing Strategy Call — we’ll talk through your marketing needs, what we offer, and your pricing.
If it’s a good fit, we’ll set up a second call to go in-depth on what working together looks like, after which you’ll receive a custom proposal.
Once you sign, we’ll get your kickoff call on the books right away and start building your new marketing funnel.
